Request form for Sexuality Education in your Classroom

• This form is for junior and senior high school presentations only within the Calgary Region. All topics relate to the Alberta Education Curriculum.
• Education sessions can only be booked for Tuesdays, Wednesdays, and Thursdays.
• Requests are dependent on educator availability. Generally, we can accommodate up to 2 sessions, on 2 different topics, per group. Below is a list of the topics we offer.
• We usually require a minimum of 4 weeks notice. You will be faxed a confirmation of your sessions once they have been booked.
•  Only completed forms will be considered for bookings. Incomplete forms will be returned.
• Requests can be submitted via email or fax by using the electronic form found here.
